DVR Certification of Criminal Background Checks

Certification of Criminal Background Checks: Certification regarding criminal background checks on all Service rovider personnel or contractors who work with Division of Vocational Rehabilitation (DVR) consumers or work with DVR consumer information.

  1. The Service Provider certifies that it has read and agrees to comply with the DVR Criminal Background Check Policy.
  2. The Service Provider certifies that it has conducted the required criminal background check for all service provider personnel or contractors who work with DVR consumers or who work with DVR consumer information, including individuals serving as the main contact for any service agreement and individuals who perform a service provider's reporting or billing related to its service provider agreement.
  3. The Service Provider certifies that it has reviewed the Wisconsin Sex Offender Registry to ensure that staff and subcontractors who provide services to DVR consumers or work with DVR consumer information are not on the offender list.
  4. The Service Provider certifies that, after reviewing the policy and performing the required checks, it is in compliance with the requirements of DVR's Criminal Background Check policy.
