The following tables summarize the reports and forms associated with DVR Statewide Services and Other Services. These tables are intended to provide a quick reference for required documentation, including report names and links, the purpose of each report, submission expectations, and the services to which each report or form applies. Service Providers are responsible for ensuring that all reports and forms submitted to DVR meet the requirements outlined in the Technical Specifications for the applicable service.
