As a state and federally funded program, the Division of Vocational Rehabilitation (DVR) must collect certain documentation from consumers receiving DVR services. The information you provide is used to confirm your involvement and progress with DVR services. DVR will remind you when different information is needed and will help you figure out the best way to get it.
You may send DVR the requested information by fax, text message, email, mail (in a self-addressed envelope from DVR), or sometimes verbally either in-person or by phone.
If you are under 18 years of age or have a legal guardian, please share this information with them. Below is a partial listing of documents that may be required for your individual case:
