Description: Use this form when making adjustments or corrections to Wage Records.

Comments: The Wage Adjustment is used to adjust or correct wages previously submitted. This should be completed with the Employer UI ten digit account number, Social Security Number, Last Name and First Name, Wages Reported and Correct Wages. This form can be used to submit wage adjustments for up to four quarters in a single calendar year.
