Individual Placement and Support (IPS) is an evidence-based model of Supported Employment for consumers with the most significant disabilities (Category 1) who have a primary diagnosis of serious and persistent mental illness or substance use disorders and need long-term support to maintain a job. IPS services are provided in a working alliance with mental health services and include planning for employment, job development, job placement, on-the-job support, transition to long-term support, and successful employment. To build ongoing collaboration, DVR attendance at meetings and continued use of information from the IPS Employment Center is encouraged, including resources on implementation of the IPS model, agency guidance, fidelity reviews and recommendations, policies, and regulations.
IPS services are intended to use principles found at the IPS Employment Center to maximize consumer independence and choice in support.
Service providers that have agreed to provide IPS Supported Employment services also agree to have qualified personnel complete the services and DVR-required reporting for services funded through DVR. IPS service providers must meet all qualifications outlined by their county mental health agency and the Wisconsin Department of Health Services.
Before services can be authorized, IPS services must be approved statewide. IPS services cannot be provided until the service provider has made initial contact with the DHS IPS Mailbox at email@example.com and the approval process has been initiated. When a county indicates interest in IPS services, the IPS state team works with the DVR Contract Specialist and the local DVR Workforce Development Area (WDA) Director to ensure the service provider is approved and IPS services are added to their service contract.

The Career Profile is a report in which the service provider records work preferences, work history, education history, strengths, justice system involvement, and other information pertinent to a consumer’s employment or education goals. Using this information, the service provider should develop an action plan for job development honoring the stated preferences of the consumer.
The Career Profile is intended to be completed over several face-to-face meetings with the consumer and others who may be helpful in assisting in a job search or providing support once the consumer obtains employment. The Career Profile should identify a team of individuals who provide support to the consumer, and may have information about the consumer's employment or education history. It should contain detailed information and reflect the choices, preferences, and interests of the consumer.
The service provider should use the information gathered in the Career Profile to consult with the assigned DVR IPS representative, the consumer, and any natural supports identified by the consumer, to determine if the Career Profile contains the information necessary for the DVR process to move forward.
It is expected that the Career Profile will be used as a “living” document and added to as needed to assist in meeting the consumer's employment and support needs. The Career Profile will be reviewed, updated, and shared with DVR at least once every six months.
It is possible that work on the Career Profile will begin before DVR involvement in the case as a Career Profile is a required component of IPS. If the Career Profile is near completion, DVR may choose not to authorize the service and instead allow the service provider to bill Medicaid for the service. DVR will work with the service provider to determine if the Career Profile is near completion.
The Career Profile form is to be completed by the service provider with a consumer during the initial months following referral to the IPS Employment Specialist. Sources of information include the consumer, the mental health treatment team, client records, and with permission, family members, other natural supports, and previous employers.
The Career Profile should be updated with each new job experience and identify strategies for job retention, including detailed information that reflects the voice of the consumer.
The IPS Job Development Plan service requires a detailed Job Development Plan and resume. Information gathered in the Career Profile should assist in job development efforts. The Job Development Plan should be reviewed with DVR and the consumer every 60 days in-person or by phone or virtually. A case note confirming that the Job Development Plan has been reviewed is required by DVR if the meeting is not in-person. Changes can be made as needed in full consultation with all parties.
As part of Job Development, the service provider will provide employment preparation services (interview preparation, resume development, visual resumes, etc.) as identified in the planning process when Job Center services do not adequately address the consumer's needs.
During the initial phases of the job search, it is important for the service provider to discuss disclosure of disability with the consumer. Consumer preferences for disclosure are specific to the individual and may change over time. The service provider should confirm these preferences with the consumer regularly and record them throughout the process.
Internship/Temporary Work Experiences or other work assessments are generally not accepted as part of model IPS services, but may be allowed by DVR in unique consumer situations. DVR approval is required.
IPS fidelity states that a job supports plan should be completed for each job seeker. In addition to the Supported Employment Job Search Plan, the service provider may complete the Initial Job Supports Plan with the consumer and support team (or a different form of the service provider's choice) to document conversations about what initial supports might look like for the consumer while working. The purpose of the IPS Initial Job Supports Plan is to help the consumer begin thinking about common barriers to working, including steps to manage stress and the tools necessary to do their job.
The IPS Systematic Job Development service requires monthly job development progress reports. As noted in the IPS Employment Center best practices, job development is conducted in IPS by building relationships with employers. Service providers may contact employers in a more general sense, but should still reach out to potential employers based on individual consumers' stated areas of interest.
Service providers should work cooperatively with DVR Business Services Consultants to maximize potential job leads and relationships with local businesses.
Service providers must conduct meetings with the consumer, the IPS Employment Specialist, and DVR at least every 60 days to review the Job Development Plan and verify goals, business contacts, strategy, etc. If necessary, the Job Development Plan will be amended if all parties agree.
IPS Systematic Job Development should include assisting the consumer to achieve the highest wages possible and find the best job match given the consumer's strengths, abilities, capabilities, informed choice, concerns, priorities, interests, and resources. IPS Employment Specialists should work with the consumer to ensure that there is a good job match and that the job has opportunities for career advancement.
IPS Employment Specialists can assist businesses in identifying, modifying, and eliminating environmental barriers and provide recommendations to DVR for needed assistive technology consultations or assessments. DVR may also provide technical assistance to businesses as appropriate.
Upon request, IPS Employment Specialists, in cooperation with DVR staff, will educate businesses about disability related issues, including relevant legislation.
DVR will provide information and support to a consumer to maximize the number of hours the consumer can work. A position obtained by the service provider must be consistent with the consumer's employment goal listed in their IPE. If the position obtained does not reflect the IPE goal, a documented conversation must occur to amend the IPE if all parties agree. During initial and ongoing meetings, the employment team, including the consumer, should discuss the number of hours the consumer would like to work and include this information in the Job Development Plan report. This estimate of hours should be based on the consumer's stated preference and ability to work.
Upon an offer of employment, DVR should be notified as soon as possible (within 5 days) to authorize Job/Task Analysis and Systematic Instruction services for monthly supports if needed.
After the consumer begins employment, the service provider should complete the second part of the IPS Initial Job Supports Plan (or similar job supports planning form). This section outlines responsibilities for the consumer, service provider, systematic instructor, care coordination team, and DVR to provide work-related support to the consumer. This section also addresses wage reporting to the Social Security Administration (SSA), disclosure preferences, and potential need for future benefits counseling (Work Incentive Benefits Analysis services).
Service providers should obtain wage documentation between the time of hire and transition to Long Term Support. The types of acceptable documentation are detailed in the Wage Verification Options announcement.
A discussion with the service provider, consumer, DVR, and long-term support provider (and any others invited by the consumer) is required to secure an appropriate plan and time to facilitate the transition to long term support services. A signature from all parties (consumer, service provider, Wisconsin county Mental Health representative, and DVR) is required to confirm and document agreement. A meeting is not required; however, DVR must document that all parties have agreed on a plan to provide support to the consumer and a definition of what stabilization will look like based on the consumer's individual circumstances.
If the service provider has provided substantial services (documented in the Systematic Instruction Monthly Skill Instruction Reports) and has aided significantly in the transition to LTS with an acceptable report, they may receive payment for this service at the discretion of the local DVR office at the time of the transition to long term supports.
Service Providers are not required to provide monthly reports to DVR after the transition to LTS. If the DVR case file is not closed, DVR can continue to contact the consumer, employer, or long-term support provider to obtain information for case closure purposes.
WIOA states that DVR is required to follow the consumer for an additional 90 days after successful transition to LTS before closing the case.

Monthly rate payable if the consumer is employed during the month, upon completion of acceptable service, and timely monthly progress reports to DVR within 5 business days of the conclusion of the previous month of service. Initiation of the service not to exceed 30 days from issue of service authorization.
Note: The maximum ongoing support DVR can pay for is 24 months for anyone age 25 or above and 48 months for individuals under age 25, although this is rarely needed. Additional months may be authorized based on individual consumer need to establish job stability and reduction of paid supports. If a consumer does not have an identified source of long term supports, monthly supports and continued reporting will be required until the consumer is transitioned to natural supports or as directed by DVR.
|$1200||Individual Placement and Support (IPS) Transition to Long Term Supports and Report:
Paid only if transition occurs within initial six (6) months of support. This service cannot occur until after the consumer has been employed for a minimum of 90 days. Payable at the time of successful transition. Per DVR supported employment policy, DVR will keep the consumer's case open an additional 90 days for purposes of follow-along support. Transition to LTS service payment can be processed upon receipt of an acceptable report.
